As economies scramble to evolve payment, quality, and access issues, there’s a widespread movement towards value-based purchasing. As a result, hundreds of population health models exist around the world designed to streamline patient care, reduce adverse events, avoid readmissions, and improve outcomes. And, healthcare providers share a common struggle in identifying and implementing processes, applications, tools, technologies, and systems necessary to achieve these goals. A pivotal component of this transformation is population health management; it is rapidly emerging as a required core competency in this developing environment. With advances in the use of connected health technologies --- remote patient monitoring and new care models leveraging telehealth --- the patient will have an increasingly dynamic role in their own care as providers engage on a real-time basis to manage their overall experience.
